Make A Referral

Are you wondering about how your child plays, learns, speaks, or acts? Would you like more information on parenting? Are you challenged by your child’s behavior? Are you interested in connecting with community services for your child? 211 Child Development helps support children’s healthy growth and development starting during pregnancy. 

To make a referral, call 1-800-505-7000 or fill out one of the forms below. Please make a separate referral for each child and/or pregnancy.

By speaking with a 211 Child Development care coordinator, families can:

  • Learn about the skills children usually develop as they grow
  • Get information and support on questions or concerns about a child’s development or behavior
  • Track your child’s growth and milestones with the Ages and Stages Child Monitoring Program
  • Get referrals to community programs and services that help support children and families
  • Connect to family resource centers, play groups, recreational activities, home visiting programs, and more

If you have questions or concerns about a child’s development, behavior, or milestones, you can make a referral to Birth to Three. A parent, foster parent, DCF worker, or anyone else can make a referral. By clicking on the referral form, you will be redirected to the Birth to Three website.

Care coordinators can share information about pregnancy, health care, home visiting programs, childbirth and parenting classes, healthy food programs, and other helpful resources.

Home Visiting Programs provide support in the home and promote positive parenting and healthy development. Some different types of home visiting programs include Parents as Teachers, Child First, Nurse-Family Partnership, and Healthy Families America.
