Help Me Grow Model
Help Me Grow connects families to the resources they need, when they need them.
The early years are a critical time for development, but families often struggle to find and access support. Help Me Grow strengthens the system by linking community services, supporting providers, and helping families navigate care for their child’s development and well-being.
The Help Me Grow Model has 4 Core Components:
- A Centralized Access Point assists families and professionals in connecting children to the grid of community resources that help them thrive.
- Family & Community Outreach builds parent and provider understanding of healthy child development, supportive services available to families in the community, and how both are important to improving children’s outcomes.
- When potential concerns are spotted early on, they can be easier to address. Child Health Care Provider Outreach supports early detection and intervention efforts and connects medical providers to the grid of community resources to best support families.
- To make sure the resource grid is working effectively, Data Collection & Analysis supports evaluation, helps identify systemic gaps, bolsters advocacy efforts, and guides quality improvement
Help Me Grow Council
The Help Me Grow Council is composed of a diverse early childhood workforce across various agencies and programs. The advisory group meets monthly from September to June to learn about a specific topic, network, and to discuss strengths and challenges in a solution-based approach towards early detection of all children in our state. Help Me Grow National Center
The Help Me Grow National Center is dedicated to ensuring every young child has the chance to reach their full potential by supporting the growth and excellence of Help Me Grow systems across the country.
The National Center strengthens the work of affiliates nationwide by:
- Sharing lessons learned across the Network so success in one place benefits all.
- Creating and refining tools that help both new and established systems implement the Model effectively.
- Introducing timely innovations that keep systems responsive and relevant.
- Raising the visibility of Help Me Grow as a trusted solution for early detection, referral, and connection to services.
Help Me Grow National Forum
Help Me Grow National hosts an annual forum that brings affiliates and partners from across the country together to share innovations, build connections, and strengthen our collective impact.
